Expected Outcomes

• Validated, scalable multimodal–data solutions that improve personalised management of multimorbid patients
• Demonstrated reduction in time to diagnosis, improved monitoring of disease progression and relapses, and enhanced detection of co-morbidities
• Effective remote monitoring and reporting workflows that enable a shift from in-patient to out-patient care and reduce healthcare utilisation
• Decision-support tools for optimised treatment strategies, including drug selection, dosing and management of polypharmacy, leading to fewer adverse interactions
• Increased patient adherence to long-term regimens through data-driven monitoring and support
• Holistic system models demonstrating integration of technical, organisational, regulatory and behavioural components
• Robust data pipelines and governance frameworks for secure, interoperable use of multi-modal health data
• Business and value models outlining paths to commercialisation and health system adoption
Funded areas

Scope Requirements

• Target population: multimorbid patients (≥2 chronic diseases requiring management)
• Focus on use of multimodal data (clinical, wearable, imaging, omics, patient-reported, etc.) to support personalised interventions
• Address at least one of the following: detection/characterisation of co-morbidities; disease diagnosis, follow-up or monitoring; remote monitoring for out-patient care; decision support for treatment strategies; multi-treatment management; adherence support
• Solutions must be at Technology Readiness Level (TRL) sufficient for testing in controlled real-world settings (i.e., mature prototypes or pilot deployments)
• Must adopt and integrate all six enabling system elements
• Proposals must detail a clear plan for data acquisition, integration, processing, analytics, interoperability, privacy/security and ethical compliance
• Transnational consortium must include at least one clinical partner and at least one SME or industry partner
• Clinical studies are eligible; large-scale clinical trials are out of scope
Additional Conditions for Applicants

Special Conditions

• Mandatory inclusion of a clinical partner plus at least one SME or industry partner in each consortium
• Large clinical trials (e.g., Phase III trials) are excluded; only smaller clinical studies and pilot evaluations permitted
• Solutions must be tested in controlled real-world environments (e.g., pilot sites, clinical settings, remote monitoring scenarios)
• Projects must integrate the six predefined enabling elements holistically
• Proposals must demonstrate compliance with data protection, ethical, regulatory and interoperability standards for health data
• Projects should align with the EP PerMed Strategic Research & Innovation Agenda (SRIA for PM 2023)
• Funding is under Horizon Europe (Grant Agreement N° 101137129); national/regional co-funding rules of participating agencies apply
• Pre-proposal deadline: 12 January 2026; full proposal deadline: 27 April 2026; project start early 2027
